A jet that crashed at Bangor International Airport on Jan. 25 may have waited too long to take off after deicing, according to a preliminary report from the NTSB

The crash killed six people, including the pilot, co-pilot and four passengers, the NTSB said

The jet “sustained substantial damage” after it crashed back to the ground just moments after attempting to take off, the report noted

A preliminary report into the Maine plane crash that killed six people suggests an eight-minute mistake may have been behind the fatal incident.

The Bombardier Challenger 650 attempted to take off from Bangor International Airport about 17 minutes after deicing began on Jan. 25, according to the National Transportation Safety Board (NTSB) report issued Friday, March 6, obtained by PEOPLE.

But Federal Aviation Administration guidelines state the plane should have waited no longer than nine minutes from the start of deicing before taking off, the noted.

In a cockpit voice recording, the pilot said it was “standard” to have 14 to 18 minutes of holdover time, adding that the jet would return to the ramp to deice again if the wait to take off “was more than 30 minutes,” to which “the copilot concurred,” according to the NTSB report.

The jet “sustained substantial damage” after it crashed back to the ground just moments after attempting to take off, officials said in the report. The official cause of the crash has not been released at this time.

The pilot, the copilot and the four passengers who were onboard the plane at the time of the incident were killed, according to the NTSB.

The victims were previously identified as Captain Jordan Hosmer, Tara Arnold, Shawna Collins, Nick Mastrascusa, Shelby Kuyawa and Jorden Reidel. Officials previously said extreme weather conditions affected the progress of the investigation, and it took several days to retrieve the victims from the crashed aircraft.

Hosmer, 47, was a pilot from the Houston area, according to NBC affiliate KPRC. He had worked for Houston-based injury law firm Arnold & Itkin LLP since May 2025, according to his LinkedIn.

The plane involved was registered to a company with a Houston address shared with the personal injury law firm Arnold & Itkin, Reuters previously reported.

Tara was the wife of Arnold & Itkin founder Kurt Arnold, according to ABC affiliate KTRK. Neither Kurt nor his firm’s co-founder Jason Itkin were onboard the plane at the time of the crash.

Collins was an event planner, according to her daughter, who reportedly spoke with her mom prior to the crash about the European business trip Collins was going on.

Mastrascusa was a father of three, according to a GoFundMe, as well as a chef who worked in some of the "world's most exclusive restaurants and resorts," KTRK reported.

Kuyawa, 34, was a sommelier from Hawaii, while Reidel, 33, was the second pilot on board the aircraft, according to ABC News.
